Insurance requirements vary from provider to provider. For specific requirements, please contact your employer or call your insurance company directly. The following are general requirements that apply in most cases.

• A pre-certification is required for all Bone Density studies, MRI's, MRA's, CAT Scans and Ultrasound studies prior to scheduling the study.

• Cardiac Scoring, Lung Screening and Total Body Scanning are not covered by insurance.

• In cases of auto accidents, the auto insurance company may require pre-certification of treatment when ordered 10 days after the auto accident.

• Workers' Compensation must be pre-authorized.
